Dale LaChance, PMHNP
Psychiatric Nurse Practitioner
Dale LaChance is a board certified psychiatric mental health nurse practitioner (PMHNP). He has worked in numerous hospital and outpatient settings and spent 10 years overseeing the daily operations of the psychiatry, medical, and dental services at a state juvenile correctional facility.
He believes in evidence-based practice and a holistic approach to mental health, and has a comprehensive background working in private, rural community outpatient mental health clinics, and foster care agencies providing medication management and mental health care to children, adolescents and adults.
He specializes in treating ADHD, ODD, depression, anxiety, bipolar disorder, and schizophrenia. Dale graduated with a Bachelor’s of Science in Nursing from Kent State University and completed his Master of Science degree for Psychiatric Mental Health Nurse Practitioner at Walden University.

Patrick Testa, PMHNP
Psychiatric Nurse Practitioner
Patrick Testa is a board certified psychiatric nurse practitioner licensed to diagnose and treat mental health conditions. He has experience helping patients with anxiety, depression, bipolar disorder, eating disorders, ADHD, and other psychiatric issues. He went into the psychiatric field to support patients in their recovery and help break the stigma of mental illness.
In his practice, Patrick uses psychiatric medication and complementary approaches such as lifestyle changes, nutrition, and mindfulness. His philosophy is to treat the whole person and prescribe medication thoughtfully, considering best practices along with patient preferences. He enjoys working together with clients to develop a plan of care that is tailored to their goals, unique strengths, and life experiences.
Patrick graduated with his Master of Science in Nursing with a concentration in Psychiatric Mental Health at the Ohio State University. He also holds a Master’s from the University of Michigan School of Public Health. Patrick completed his clinical training across inpatient, residential, and outpatient mental health settings. In his free time, he enjoys playing tennis, hiking, reading, and spending time with his wife, friends and family, and dog.

Madeleine Sanford, PMHNP
Psychiatric Nurse Practitioner
Madeleine Sanford is board certified as both a family and psychiatric mental health nurse practitioner (FNP-C, PMHNP-BC). After extensive experience in family medicine in multiple settings including outpatient, school-based health, education, and urgent care, she obtained an additional degree to become a PMHNP and focus on her passion for mental health.
Her style is individualized to the person, evidence based, trauma informed, and holistic. She sees patients across their lifespan, including children and adolescents. Additionally, she has completed training for medication-assisted therapy for opioid use disorders (MAT).

Emily Charlton, FNP-C
Family Nurse Practitioner
Emily Charlton is a board-certified family nurse practitioner who recently returned to graduate school to further her education as a dually certified psychiatric mental health nurse practitioner. She has studied at Gonzaga University and Liberty University. Her experience as a nurse extends back to 15 years ago when she started her career as a certified nursing assistant in long-term care. Previous nursing experience includes geriatric care, home healthcare, medical surgical nursing, and outpatient chronic pain management. Her advanced practice nursing experience includes primary care, family medicine, and urgent care.
She is an empathetic nurse practitioner who is focused on providing quality care and maintaining direct lines of communication with patients and the healthcare team. Emily is a calm and understanding professional who strives to treat both body and mind to provide care that is holistic and individualized. She cares for patients across the lifespan with various acute and chronic conditions.
Emily is a Portland Native and enjoys outdoor activities such as hiking, camping, and running. She is also a crafter, baker, lover of animals, and a strong supporter of the Portland Timbers. She enjoys spending time with her dog, Coraline, at the park and spending time with her sisters and two young nephews.

Marina Banderas
Community Health Worker
Meet Marina, our dedicated and bilingual Community Health Worker at Well Life Family Medicine. With proficiency in both Spanish and English, Marina is a registered professional through the Oregon Health Authority. She finds immense joy in assisting individuals in accessing the resources necessary for their well-being.
Marina is passionate about helping people achieve their goals, and her role at Well Life Medicine allows her to actively contribute to the success of our patients. Her approach to working with individuals is rooted in the belief that each person is the expert of their own experiences. With this philosophy in mind, Marina strives to build a strong connection with patients while providing unwavering support.
Beyond her work, Marina enjoys spending quality time with her beagle companion. Her love for reading and decorating further enriches her life outside the clinic.
