Emily Charlton, FNP-C & PMHNP
Family Nurse Practitioner & Psychiatric Nurse Practitioner
Emily Charlton is a board-certified family nurse practitioner who recently returned to graduate school to further her education as a dually certified psychiatric mental health nurse practitioner. She has studied at Gonzaga University and Liberty University. Her experience as a nurse extends back to 15 years ago when she started her career as a certified nursing assistant in long-term care. Previous nursing experience includes geriatric care, home healthcare, medical surgical nursing, and outpatient chronic pain management. Her advanced practice nursing experience includes primary care, family medicine, and urgent care.
She is an empathetic nurse practitioner who is focused on providing quality care and maintaining direct lines of communication with patients and the healthcare team. Emily is a calm and understanding professional who strives to treat both body and mind to provide care that is holistic and individualized. She cares for patients across the lifespan with various acute and chronic conditions.
Emily is a Portland Native and enjoys outdoor activities such as hiking, camping, and running. She is also a crafter, baker, lover of animals, and a strong supporter of the Portland Timbers. She enjoys spending time with her dog, Coraline, at the park and spending time with her sisters and two young nephews.

Denise Tobe, FNP-C & PMHNP-BC
Family Nurse Practitioner & Psychiatric Nurse Practitioner
Denise Tobe, FNP-C, PMHNP-BC, is a board-certified family and psychiatric mental health nurse practitioner with special interest in anxiety and obsessive-compulsive disorder. She earned her Master of Science in Nursing from Grand Canyon University of Arizona and completed her post-master’s psychiatric training at Regis College in Boston, Massachusetts. Denise encourages her clients to take control of their physical and emotional health through their environment and relationships. She works to understand each person’s goals and values, and approaches treatment with flexibility and curiously rather than a rigid protocol. Her goal is to support people in feeling grounded, confident, and connected as they move toward better mental health. She values honest collaboration and believes care works best when clients feel heard and respected.
Outside of work, Denise enjoys spending time with her husband, their three daughters, her toy poodle Ollie, and their mischievous Cornish rex cat, Lucy. She’s a fan of the New York Yankees, loves everything Halloween and horror movies, and enjoys catching a live classic rock band-still hoping Hall and Oates make peace and tour together again.

David Ramsay, LPC

David Ramsay is a Professional Counselor Associate at Well Life Medicine, having graduated with honors from Washington State University for Psychology and with honors from Lewis & Clark College with a Masters in Counseling. His approach to therapy is to first respect and honor that each person is truly unique, with their own story, identity, values, and perspectives; lives are more than a label or diagnosis, they are complicated, each with various strengths and challenges, resources and obstacles.
David is committed to truly hearing and understanding the client and the issues of their life. He aims to offer support, guidance, and judgment-free space to explore clients’ concerns and challenges while furthering their own self-understanding and ability to self-advocate, building and capitalizing on strengths, while collaboratively addressing the problematic areas upon which a client wishes to improve or heal.
David prioritizes the client’s autonomy with the aim of enabling a client to become attuned to their development, well-being, and an open-ended exploration of possible life options and choices. David maintains an eclectic theoretical orientation with clients, often utilizing various cognitive behavioral approaches that are informed by mindfulness. David abides by the Code of Ethics of the Oregon Board of Licensed Professional Counselors and Therapists and those of the American Counseling Association.
David is a devoted husband and father and has loved all the opportunities for camping, hiking, and mountain biking in the Pacific Northwest.

Dale LaChance, PMHNP
Psychiatric Nurse Practitioner
Dale LaChance is a board certified psychiatric mental health nurse practitioner (PMHNP). He has worked in numerous hospital and outpatient settings and spent 10 years overseeing the daily operations of the psychiatry, medical, and dental services at a state juvenile correctional facility.
He believes in evidence-based practice and a holistic approach to mental health, and has a comprehensive background working in private, rural community outpatient mental health clinics, and foster care agencies providing medication management and mental health care to children, adolescents and adults.
He specializes in treating ADHD, ODD, depression, anxiety, bipolar disorder, and schizophrenia. Dale graduated with a Bachelor’s of Science in Nursing from Kent State University and completed his Master of Science degree for Psychiatric Mental Health Nurse Practitioner at Walden University.

Emani Cheng, PMHNP

Emani Cheng is a culturally sensitive psychiatric nurse practitioner with a passion for providing quality mental health care to individuals from diverse backgrounds. With six years of experience in the psychiatric field, Emani has developed unique skills and expertise that allow her to offer comprehensive and compassionate care to her members.
After completing her undergraduate degree in nursing, Emani went on to obtain a Master’s and Doctorate in nursing practice, specializing in psychiatric mental health. She has since worked in a variety of settings, including hospitals, community health clinics, and private practices, where she has provided care to members with a wide range of mental health conditions. Specializations Include: Anxiety, Depression, Bipolar Disorders, Mood Disorders, Psychotic Disorders, and Crisis Intervention

Madeleine Sanford, PMHNP
Psychiatric Nurse Practitioner
Madeleine Sanford is board certified as both a family and psychiatric mental health nurse practitioner (FNP-C, PMHNP-BC). After extensive experience in family medicine in multiple settings including outpatient, school-based health, education, and urgent care, she obtained an additional degree to become a PMHNP and focus on her passion for mental health.
Her style is individualized to the person, evidence based, trauma informed, and holistic. She sees patients across their lifespan, including children and adolescents. Additionally, she has completed training for medication-assisted therapy for opioid use disorders (MAT).
